Hillingdon Community Diabetes Team have a vacancy for a full time Diabetes Specialist Dietitian at Band 6 to join our amazing community team of diabetes specialists’ nurses, Dr, consultant and dietitians.

Hillingdon Community Diabetes Team is a diverse multi-disciplinary team that works in primary care to design care plans that meet the needs of people with Diabetes. The Hillingdon CCG team are part of the wider NHS West London Clinical Commissioning Group (CCG). The team serves patients registered with a GP in the Hillingdon borough. There are over 40 GP surgeries and 7 Primary Care Networks (PCN). Our community clinics are based across the borough and at our team base in the HESA center in Hayes town. Diabetes can have a significant impact on the quality of someone’s life and the financial implications on the NHS. The Team aims to optimise health and wellbeing, reducing complications and prevent unscheduled hospital visits.

The post holder will be interested to develop their skills in Diabetes. They will work autonomously without direct supervision, to deliver evidence-based care, education, and information to people with diabetes and their families referred to the Hillingdon Community Diabetes Team. They will demonstrate and understanding and commitment to providing all patients with Diabetes the help they need to identify what is important to them, what they would like to achieve and how they plan to achieve it. Post holder will demonstrate that they have the attitudes and behaviours required to support people to make and sustain lifestyle changes.

The post holder will provide specialist clinics, structured education programs for adults with diabetes and training programs for health care professions.
